This notice explains how Lifeline Guidance handles personal information and personal health information in connection with psychotherapy services in Ontario.

Information we collect

We collect only the information reasonably needed to respond to inquiries, arrange and provide care, maintain clinical and administrative records, process payment, issue receipts and meet legal and professional obligations. This may include contact details, appointment and billing information, consent documentation, clinical notes and information you choose to share with your therapist.

Please avoid sending detailed clinical information through ordinary email. Secure intake and booking functions are provided through the practice’s booking and record platform.

How information is used

Information is used to provide and coordinate care, communicate with you, administer appointments and payment, maintain accurate records, support quality and safety, and meet regulatory or legal requirements. Consent is sought as required, and information is not used for unrelated marketing without an appropriate choice.

When information may be shared

Personal health information is not disclosed outside the purposes of care without consent unless disclosure is permitted or required by law. With your knowledge and consent, a therapist may coordinate with another healthcare professional. Limited information may also be handled by service providers that support secure scheduling, records, communications, payment or website hosting.

Your therapist will explain the limits of confidentiality during informed consent, including circumstances involving risk of serious harm, child protection, legal requirements or regulatory processes.

Safeguards and retention

Lifeline Guidance uses administrative, technical and physical safeguards appropriate to the sensitivity of the information. Access is limited to people who need information for an authorized purpose. Records are retained and securely disposed of in accordance with applicable law and professional requirements. No electronic system can be guaranteed completely secure; suspected privacy incidents are assessed and addressed under the practice’s privacy process.

Your choices and rights

You may ask how your information is handled, request access to your personal health record, request a correction, or withdraw consent where the law permits. Some requests may be subject to legal exceptions, identity verification, reasonable timelines or permitted fees. A withdrawal of consent cannot reverse a disclosure already made with valid consent and may affect the care we can provide.

Your inquiry will be directed to the practice’s privacy contact for acknowledgement, identity verification where needed, and a response under the applicable process.

If a concern is not resolved, you may contact the Information and Privacy Commissioner of Ontario. This public notice may be updated as the practice, technology or legal requirements change.
